Our first presentation will be by Jessica Lea, CEO and Founder of Tria Health. Tria Health provides personalized pharmacy care to members of self-insured employers and health plans. By putting pharmacists at the heart of chronic condition management and applying patient-centered approaches and smart technologies, Tria Health is improving patient outcomes and driving down the cost of care.

Our second presenter will be Dale Gray, CEO of First Call Technology. First Call’s Community CareLink is a case management solution founded on a certified electronic health record. CareLink goes beyond traditional case management systems to embrace a social determinants of health model that connects the entities involved in integrated continuity of care – people service agencies, government, and funders. Within this model, users are able to electronically build out and manage an extensive network of partnering agencies for population health in what First Call refers to as its Social Health Access Referral Program (SHARP). This idea of a social health referral platforms currently is a very hot topic among payers and providers in the Kansas City region.

Every month, we bring together a group of health care stakeholders to learn and share about technology and innovation projects happening in the region–and occasionally related topics from outside the Greater Kansas City area. Our stakeholders include hospitals and the health care system, city and state government officials, health IT firms and other corporate partners, startups working in health and wellness, and other civic organizations. The meetings are open, though not widely broadcast. They generally happen on the last Wednesday of the month, 8:30-10:00 am at Mid-America Regional Council (MARC).
